The Guided Living model is for Core Members who need 3-5 hours of daily support through prompts, reminders, and some modeling of skills and behaviors.
CASS is currently in the process of developing this model, but take a look at the guidelines below for an idea of who may be successful at this level of support.
Guided Living is designed for Core Members with the following combination of abilities and support needs:
Able to handle toileting on their own
May need to be prompted to shower, but is able to handle bathing on their own
May require prompting or modeling for self-grooming
Able to feed themselves and portion their own servings
Able to grocery shop with minimal assistance, plan meals with guidance, and make pre-made or basic meals
Able to use public transportation but may need assistance to organize scheduled trips
Able and willing to take any necessary medication on their own with minimal reminders
May need prompting and reminders to be respectful of personal space
Able to identify daily basic needs, but may need help advocating for themselves
Able to identify who to reach out to for help, needs, and emergencies
The support provided to Core Members at the Guided Living level includes 5 hours of Steward support Monday through Friday (2-3 hours in the morning, 2-3 hours in the evening), prompting and reminders for daily living skills, support with meal prepping, assistance with transportation planning, and access to CASS staff.
